Family Health & Wellness Ages 12+ The University of Texas at Austin and YMCA of Austin have partnered to develop and test a bilingual (English and Spanish) type 2 diabetes self-management program that includes the whole family. This research study is investigational and will test whether participation in the ¡Salud, Salud! program can improve blood sugar levels and health-related quality of life for individuals with type 2 diabetes. Study participants will learn to manage their Type 2 diabetes by: MONITORING BLOOD SUGAR REGULARLY MAKING HEALTHY FOOD CHOICES BEING ACTIVE AS A FAMILY REDUCING STRESS Every family that participates will receive a free 6-month YMCA family membership and personal training consultation. The purpose of the study is to develop and test a family-centered type 2 diabetes self-management education and support program that will improve blood sugar levels and health-related quality of life for families from minority backgrounds. We are doing this study because we know diabetes self-management programs help people, however we do not know enough about how to successfully deliver these programs for minority families in community settings. This study will help us identify a program approach that will improve the health of families from minority background living in Austin, TX. What will happen if I take part in this study? If you choose to participate, you will be randomly assigned to one of two diabetes self-management programs, each of which are three months long. Group 1 Group 2 YMCA Family Membership Program Orientation with a personal trainer Nutrition education materials Text messages YMCA Family Membership Program Orientation with a personal trainer Nutrition education materials Text messages Online education modules Support from a Community Health worker Nutrition education session This study will take place over 7 months. You will also be asked to complete 4 assessments. Each assessment is completed in 1 to 2 visits over a 1-week period. You will complete assessments at: Baseline (before starting the ¡Salud, Salud! program) Month 3 (after finishing the ¡Salud, Salud! program) Month 6 (three months after the program) Month 7 (four months after finishing the program) Where will the study occur? The four study assessments will be completed in person at one of two YMCA of Austin centers: COA/North Austin YMCA (1000 W Rundberg Ln, Austin, TX 78758) East Communities YMCA (5315 Ed Bluestein Blvd, Austin, TX 78723) The YMCA membership will be valid at any of the 8 facilities in Travis and Hays Counties. If you choose to participate and are assigned to Group 2 diabetes self-management program, all Community Health Worker and Nutritionist visits can be held in person (at COA/North Austin YMCA or East Communities YMCA), by telephone or via videoconference.
